Apply for Mobility Services

Eligibility

Mobility Services is available to people who cannot use conventional transit for all or part of their trip. Eligibility is based on your functional ability to use conventional transit.

You may qualify as:

  • Unconditional: A permanent condition prevents you from using conventional transit.
  • Conditional: You can use conventional transit sometimes, but barriers like snow, ice, or extreme cold limit access.
  • Temporary: A temporary condition prevents you from using conventional transit for a period of time.

How to apply

To apply for Mobility Services:

  1. Complete the application form, including the healthcare professional certification.
  2. Submit the form by email, mail, or in person:

Applications are reviewed within 14 business days. You will receive written notification of the decision.

If not approved, you’ll receive a letter explaining the decision and outlining how to appeal.

If approved, you’ll receive a registration number and an information package.

Appeals

If your application is not approved, you may submit an appeal with additional documentation supporting your need for Mobility Services within 30 calendar days. Appeals are reviewed by an independent panel, and a final decision will be made within 30 calendar days.

Visitors

Visitors to Guelph who are eligible for specialized transit in their home community may access Guelph Transit Mobility Services for up to four months.

To register, please provide one of the following:

  • A note from a healthcare professional
  • A copy of an approved application from another specialized transit provider
  • A letter confirming current registration with another provider

Once submitted, visitor applications are processed within 48 hours, and a temporary registration number will be issued. You will also receive an information package outlining how to use the service.

Collection and use of information

Personal information, as defined by the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA), is collected under the authority of the Municipal Act, 2001, and in accordance with the provisions of MFIPPA. Personal information on this form will be used for the purpose of determining eligibility for Mobility Services and to ensure that adequate resources are provided at the time of service.
